The form seems to be in order as far as I can see. One detail might be that
you require the submitter to give an English name, but no French or
indigenous name. There might also be the question of "name" vs "names".
The problem some times is to identify a language because the names given may
vary quite a bit. It might be a good idea to encourage submitters to give
all the possible names she/he can think of. There very often is a fine line
between "English name" and "indigenous name".
Just a thought; I am not insisting that anything need to be changed.
