I have tried all four stylesheets referenced in the cookbook and the result
is the same with all of them, just header, no index terms themselves. I had
the same problem with subject headings until I realized that it needed to be
within the broader "Index terms" tag. Is this also true of an index proper?
I had started with the template but had deleted sections and later went
added them back in when I realized I needed them. I might have missed
something somewhere.
