Marty,
I have the form below saved as a file that I can "read" (insert) into an
email message. (We use a program called Pine which has a command for "Read
File" that takes a stored file and inserts it into the text of your
email). Then I just fill it out, adding or deleting elements as
necessary, and then email it off. This way, I can just reuse the saved
blank form as often as I need, editing it each time as needed but never
changing the master copy. Once sent as email, I can make a
printout of it for my files.
