More on the rest later, but on this:
It's really a question of tools support. XSLT processors really only
understand tags and attributes. They have no ability to process
strings within elements. Now, this will change in the future with XSLT
2, but in general the principle of XML processing is that where you
can, you ought to code stuff with (XML) structure. This is why I
always harp on it; I can barely handle XSLT and don't want to have to
learn another language along with it ;-)
