Just a note to you.
I once asked my funnel liaison about this, and she said it was ok to create
ARs using the form of the name on the thesis. We do this regularly and put
them into the NAF

The thing is, once you get some other publication to catalog in which they
use a different form of name, you still have to search OCLC to see if they
have used any other forms, and you will probably come across the thesis.
Current practice is to take the form of name on a thesis into account when
determining the authorized form, and if another form of name is not used
80% of the time, you will use the fullest form, which will be the form
found on the thesis! So unless you wait until they publish a lot of
things using some other form, you might as well go ahead and set the name
up based on the thesis.
We regularly set up authority records for authors of UW dissertations and
theses, even if no cross-references are needed. When you get a variant
form, it can be added as a cross-ref at a later time.
