Without knowing the semantics of the markup, one can't assume that it
can simply be stripped away and leave plain text, e.g.
1. <title>Some <a:foo>annotation that is not actually part of
the</a:foo> title</title>
2. <title><b:img src="scan-of-the-title-page.tiff" alt="Some
title"/></title>

OK, so my hunch was right: my RELAX NG version needs to be fixed!
Thanks for the clarification.
But since I stumbled on this, how about changing the definition of
title and subTitle to allow foreign namespace elements? Having to
stick redundant information in the extension element is not exactly
ideal, and I do believe there's value in allowing for markup of this
stuff. An XSLT stylesheet could just ignore the markup.
