Subject: CBM identification label reorder requests
The three-year supply of self-adhesive chrome cassette-machine identification labels (first announced in
Network Bulletin 98-49) that NLS supplied to machine-lending agencies (MLAs) with Machines &
Accessories 02-07 is exhausted. It is now time for participating MLAs to reorder. The 2" x 4" labels are
printed in black fourteen-point type on a brushed-silver foil background. To order your labels, complete
the attached form or visit <http://www.loc.gov/nls/cbmlabelform.html> to complete the CBM Label
Request Form online.
Instructions
In states where sublending agencies (SLAs) exist, note only the MLA name and telephone number(s);
omit the MLA's mailing address. (Machines move among SLAs so frequently that it is more efficient to
provide a single point of contact for all machines.)

The MLA will supply the appropriate SLA address to anyone who calls the
number(s) on the label and thus prevent SLA-inventoried machines from being
shipped to the MLA.
In areas where no SLAs exist, provide both the MLA's address and telephone
number(s). Any MLA whose address or telephone number changes within the
next three years may obtain replacement labels directly from the manufacturer.

NLS plans to ship labels to participating libraries before the end of 2004. Please submit your order and
update the telephone numbers and addresses online at <http://www.loc.gov/nls/cbmlabelform.html> or
fax the attached order form to(202) 707-0712 by September 10, 2004. If your MLA uses locally
produced labels and does not wish to receive NLS-supplied labels, communicate this decision by
checking the first block on the order form and return as directed. We need to hear from each MLA no
later than September 10, 2004.
