Subject : IRS 2004 tax forms
The Internal Revenue Service Alternative Media Center (IRS AMC) has begun braille production
of its 2004 tax forms. As in the past, the following publications will be produced in braille for
automatic distribution to braille-lending libraries:
1040 and Instructions
1040A and Instructions
1040EZ and Instructions
Publication 17 (tax guide)
Publication 334 (small business)
Libraries that wish to receive these documents must:
E-mail a list of the publications required in braille format, along with the quantity desired for
Include the full name and address of your library
Include the name of the staff member requesting the forms
Send to Freddie Peaco, [log in to unmask], by close of business Tuesday, November 30, 2004
Libraries may also download the materials from www.irs.gov then follow the accessibility link.
Copies of downloaded/saved documents may be embossed for distribution to patrons.
As a third option libraries may provide patrons instructions for downloading the publications
directly. Patrons may complete the forms online and then print them; however, they must have
the full Adobe Acrobat software program to save them.
