Well that doesn't actually fit my understanding of the semantics of
either "server choice" or "server default" ;-)
We have three subtly different interpretations of what an omitted index
term should/could mean - none of which is covered by the three any*/all*
indexes under discussion.
Can of worms anyone ;-)
