If you wish the output to be entity encoded (rather than output ‘raw’
UTF-8) you must declare <xsl:output encoding="ascii"/>
Perhaps there is another way but I am not aware of it. That said,
there is really no reason to force your data to be entity-encoded,
unless you are going to be sending it around in email which seems
always to munge the character set.
