Sorry folks, I'm having a bad Monday and thought I'd responded to a
private message from Antony Franks, but turns out not only did I respond
to the wrong message - I sent it to this list * Oh well, sorry-  but
the information remains the same.  CPSO has been working with the
British Library, National Library of Medicine, and Library and Archives
of Canada to remove exceptional requirements for NAR creation and
modification.  Update 4, 2007 of the DCM Z1 to be published August 1,
2007 will no longer contain Appendix 2: Canadian Headings.  That
information has been condensed into 5 or 6 paragraphs and will be
included in the 1XX section of the DCM.  The requirement to consult LAC
files when establishing or modifying Canadian names will continue;
however, catalogers will be able to consult directly with LAC via a Web
form that is currently being tested by Coop.  This was announced in LC's
ALA Annual update http://www.loc.gov/ala/an-2007-update.html (under
Cataloging Policy) 

As to the examples of LAC citations I personally like Richard Moore's
examples; Sherman's and Adams' examples are exceptional as the
guidelines state that if a Canadian name is found in a heading in a
bibliographic record is not necessary to further search the LAC
authority file; therefore, either/or but not both in the same citation;
however, I know - stuff happens.  CPSO in preparation for the next
scheduled update of the DCM Z1 will take up the discussion of adding LAC
examples to the 670 section. In the meantime use your good judgement,
