Lynn, attached are two that came to me via a similar request last

The Michael Fox one is the one I based our own on, but we radically  
altered it to accommodate local markup practice. I believe there was  
a limit on levels, for example. In any case, my sense is the Apache  
FOP engine is not up to speed. I've been using an educational desktop  
version of RenderX's XEP engine (free, if you make a formal request),  
but their server version is *extremely* expensive.
