I disagree. The 400 for the surgeon should have had the qualification,
"FRCS" added to it in the first place. I believe the appropriate action
is simply to correct the 400 for the surgeon, and there is no longer any

The LCRI for 26.1 says
a) Dates. Include dates in references if they have been included in
the heading. If the form of reference conflicts with an established
heading, resolve the conflict by using dates in the reference, even if
they are not used in the heading.
So I think you're good to go.
