Hey Kevin,
According to IASA Cataloging Rules, you would use the quantity and
specific material designation "sound disc" followed by the specific
format term in parentheses. Two examples from IASA:
1 sound disc (metal mother)
1 sound disc (lacquer)

Beyond the limited format list there (Section 5.B.2.), IASA suggests
additional format terms may be applied as appropriate.
