Subject : DTBM identification label requests
Machine-lending agencies (MLAs) may now reorder chrome identification labels for
digital talking-book machines (DTBMs). The labels may be customized with the
agency's contact information, up to six lines of 14-point type. They are 2" x 3" and have
an adhesive backing that allows them to be affixed to the back of DTBMs. NLS will
provide each MLA a quantity of labels equal to its DTBM allotment for 2010.
__ Orders must be placed online at www.loc.gov/nls/dtbmlabelform.html by
April 16, 2010.__ Please observe the following instructions:
In areas where no sublending agencies (SLAs) exist, provide both the MLA's address
and telephone number. A second telephone number or e-mail address may also be
provided.

In states where SLAs exist, note only the MLA's name and telephone number. A
second telephone number or e-mail address may also be provided. Omit the MLA's
mailing address. (Machines move among SLAs so frequently that it is more efficient to
provide a single point of contact for all machines.)

MLAs may submit their requests by completing the online form at the above-mentioned
web site, which also links to Microsoft Word samples of the label format. Samples are
also provided in the attached PDF. MLAs should use these samples to ensure that their
information will fit on the label.
NLS plans to ship labels to participating libraries later in 2010. MLAs whose addresses
or telephone numbers change within the distribution period may obtain replacement
labels, if they choose, directly from the manufacturer and at their own expense.
