Just a small correction to this (and I do mean small): LC is actually calling these genre/form terms (rather than form/genre) -- LCGFT. Used in a MARC record, the code for the thesaurus would be lcgft.

Ethan, the Library of Congress encodes form/genre terms as such in its authority records (155) and has made records, at minimum, for LCFGT (LC Form and Genre Terms). Iím not sure whether theyíve made MARC21 authority records for the terms in the Thesaurus of Graphic Materials (part two is form/genre), but itís worth looking into that.
More info about LCís form/genre projects that are ongoing is here: http://www.loc.gov/catdir/cpso/genretimeline.html
