I wouldn't worry about it too much since, as you point out, there are
several different ways it's being done and I doubt there's been
confusion from any particular usage. But since you asked, I think the
best answer would be "p. 4 of cover" since there is the explicit
instruction you noted from p. 44. I don't know either what would ever
need "back cover" instead, but it's possible it was just an oversight
in the manual. It happens.

Regarding brackets, they're not necessary because the date isn't being
transcribed like it would be in the bib record. It's simply being
stated. The NACO manual instruction is on p. 42. I hope that helps.
