RCA recorded 30 ips simultaneous mono and binaural session tapes sporadically from late 1953 through early 1955. At some point in 1955, they started recording every orchestral session binaurally (the tapes were subsequently mixed to mono for LP release), and used mono for everything else. Also, sometime in 1955, they switched to recording at 15 ips. Sometime around October, 1956, they started recording orchestras 3-track on 1/2" tapes, but unfortunately stayed with 1/4" mono for all other recordings.
Why they had no backup for the April 23, 1956 session is beyond me. In the early years of binaural, (1953-54), they had only one binaural setup, so only a relatively small number of recordings were made in binaural. They would had to have had more than one by sometime in 1955, given that all orchestral recordings were recorded that way.

So, RCA was using one machine - a 2-track - for BSO sessions in 1956?? No
mono machine? Or was this not a typical session? The general impression is
that 2-track was still experimental at that time, and that mono would have
been the primary system. This situation suggests RCA had decided to embrace
stereo by early '56. But, using a mono mix of 2-track tape is so risky, even
when everything works.
(We so need a reprint of Mr. Gray's research, we who didn't read TAS. I
hereby volunteer to do the typing.)

Tom,
You might be interested to know that the Munch/Martinu Symphony No. 6, part
of the 2nd and the complete 3rd Movement are different takes in the mono
(LM-2083) and the various stereo versions. Although the entire work was
recorded in one session on April 23, 1956, the left channel failed (it was
recorded on two-track, 15ips binaural tapes) midway through the session.
The mono master used a mix for the first movement and for the last 5:23 of
the second movement. The first 1:55 of the second movement, and the
complete third movement used the right channel only for the mono LP master.
For all versions of the stereo issues, earlier in the day, previously not
chosen takes were used so that the entire recording could be issued in
stereo.

Those particular titles were issued in mono only when they were on Red Seal,
although the stereo
masters were issued later.
