I think Adam's right, but I'd also include a 411 for the form with the enumeration just so there's no confusion about whether the instance has already been established or not.

I'm sure Adam is right about establishing the individual instances according to their respective usages.
I'd look for an opportunity to wedge a hint about the numbering into a note on the heading for the collective name.
A 678, if that's the right field number.

I think you must establish each heading based on the usage of the individual conference, and it if it doesn't use numbering, then I wouldn't supply it.
