"creation" of the artifact in hand would be an printing/production
event and happening on the item level and subsequently generalized
to the manifestation level. It's a safe bet (at least for high-volume
editions or those targeted for world-wide distribution) that it took
place somewhere in India or China, the details probably not even
known to the publisher.

Thus attached to the place of publication or the "publication event"
is not "creation" but - not very surprisingly - publication. I would
expect these immaterial acts always happening at one or more specific
office of the publisher (which well might be a letterbox only).

Whether (and when) something has been published in a specific country
again has implications for copyright issues. In case of several
places given (usually as "offices" of the publisher) AACR2 required
us to record the first named place (an unamigous criterion), the most
prominently displayed place (the publisher probably would like to
express something by that distinction of one place) and the place
located in the country of the cataloging agency (just to be sure)...
