Equalizer by Brian Davies is a standalone freeware app: http://www.clickrepair.net/software_info/equalizer.html
Amadeus Pro by HairerSoft is a $60 audio editor for the Mac that has an excellent “LP / 78rpm Equalizer” plugin: http://www.hairersoft.com/pro.html
