Subject: Cassette machines returned for warranty
This month, the NLS equipment contractor, Telex, began
reporting the tracking of cassette machines returned for
warranty.
Attached is a sample copy of a list each machine lending
agency (MLA) will receive every month along with a
description of the information tracked.
MLAs should send to Telex, under separate cover, a list of
machine serial numbers and the date sent, each time
equipment is returned for warranty. MLAs should also keep a
copy of the list.
This list will assist the network, Telex, and NLS in
tracking equipment.

Description of new report sheet for C-1 warranty return.
The report will be for each machine lending agency (MLA), by
month. The report will be mailed to the address of record
for that MLA within the first week of each month.
Column 1:__ Serial Number__ of the unit sent to Telex in
Blue Earth, by the named MLA. A serial number will be
entered when the unit is received or when it appears on an
MLA list. All units received will be listed.
Column 2: __ MLA Ship Date__ . This date will be entered
from the shipping list, mailed separately, supplied by the
MLA. If no date exists, no date has been received from the
MLA.
Column 3: __ Telex Receipt Date__ . Date the unit was
received at Telex. Serial numbers and receipt dates are
entered daily. If a unit is listed on the MLA ship list and
no date is listed in this column after an appropriate period
of time, the unit may be lost.
Column 4: __ Date Repaired Start__. Date repair started
at Telex. If no date exists, the unit will continue to be
listed on subsequent reports.
Column 5: __ Date Repaired Complete__. Date repair to the
unit was completed and is ready to go through Telex QA.
This is not the date it was shipped back to the MLA.
Column 6: __ Approval Date Q.A. __ The date the unit
passed Telex Quality Assurance and can be released for
shipment to the MLA.
Column 7:__ Approval Date NLS/BPH__. NLS Quality
Assurance has the option of inspecting any repairs performed
by Telex at any time. This column will reflect such an
inspection if one is conducted.
Column 8:__ Date Shipped to MLA__. The actual date the unit
left the Blue Earth plant via "Free Matter" mailing.
Column 9: __ Date Unit Scrapped__. If a date exists in this
column, the corresponding serial number unit was scrapped
and was taken out of circulation. __ The unit will not be
returned to the MLA__.
Column 10: __ Non-Warranty Sign-Off Date__. This date
indicates the unit was out of warranty on the date it was
received at Telex. The unit was not repaired or scrapped
and was returned to the MLA. The return date will be
indicated in column 8. The sign-off date indicates the NLS
representative at the plant agreed with the out-of-warranty
status and approved return of the unrepaired unit to the
MLA.
Column 11:__ Comments__. This column will indicate any
unusual conditions. Most often, it will reflect a unit that
was not manufactured at Telex (C-2), not part of the
warranty process, or other appropriate remarks.
