It's out of band.  In practice, when you want the Bath Profile for
Z39.50, you search for "bath profile" on google and find it.  That's
how you'll find the CCG profile and/or context-set, too.  Once you've
found it, one of the things you'll find is its URI so that you can do
the necessary context-mapping.
