It seems that your system is not reading the first indicator value, or not processing it correctly. The MARC specification is quite plain and its logic pretty clear. 246 with 1st indicator = 3 ought not to generate any note; the second indicator is of significance only to the cataloguer, to justify the title added entry. Perhaps you should check whether the settings of your system for 246 indicators can be corrected. With 1st indicator set = 1, 246 should generate both a note and an added title entry. The second indicator controls the display of the caption. I can't really see why you would find a note captioned as "Parallel title" useful.
