So, it would appear that in cases of conflict, when both the fuller form and the date are available, both are used, even if only one is sufficient to disambiguate the heading.

Yes, as I said; if you have a conflict and you have both you use both.  But my point was that nearly all of the headings I found with both $q and $d did not contain both because of a conflict.
