        Subject:  Cassette book machine warranty handling
        
        
        The table attached shows the current warranty status of all recently produced or
        repaired cassette book machines by model and serial number.  Please remember the
        following items when processing under-warranty machines for repair.
        
        --     Read the warranty table carefully.  All Telex warranties from production and
       warranty repair have expired.
        
       --      Be sure that machines returned under warranty are directed to the correct
       address.
        
       --      Be sure to assess equipment carefully before returning it for under-warranty
       repair.  If a machine fails to perform properly, the machine-lending agency
       must complete an In-Warranty Defect Identification form (#FR153X) and
       include this form with the returned machine.
        
       --      Do not return machines that have suffered extensive patron abuse or that are
       obviously infested with vermin.  Report these machines "damaged beyond
       repair" to NLS BPHICS contractor for removal from your records according
       to established procedures.  These machines should be shipped for disposal as
       directed  by Jim Miller, equipment control officer.
        
       SWR, Inc., performs repairs on E-1 machines and will also repair C-1 machines
       this year.  SWR warranty returns should be sent to:

        C-1 machines repaired and under warranty by Cintrex should be returned to:
