Yes, "sculpsit" (and abbreviations) appears with names but it is more like "edited by" than "Ph. D." or "pastor of Leverhulme Parish." While my experience with looking at these statements of responsibility is more in art works than book illustrations, there is often wide variety in how an individual will sign works. Of course, the well-known artist with a wide variety of signatures is probably known to scholarship and has a nicely distinctive name with dates. It's the "Andre, sculp." that appears once that needs the help of a qualifier to distinguish him from other Andres.
