My understanding is that postnominal letters are permissible under AACR2 22.19A if they appear with the name. I agree with you that phrases, disciplines and institutions are certainly not permissible in NACO, as additions to name headings for contemporary authors, under AACR2 or the LCRI.
 
This position would be reversed under RDA, of course ;-)

I have no reluctance at all in creating or using undifferentiated names (code b), if I am unable to differentiate the name in standard ways. As far as I know it is totally acceptable within name authority and NACO practice and need not be frowned upon.  I prefer not to use  phrases, titles, disciplines, institutions, etc. that I do not feel are appropriate qualifiers.
