SUBJECT: Patron publication order form
During the next several weeks, patrons will receive large-print order forms for catalogs that were produced in 2010–2011 and those that will be produced in 2012. (We anticipate that the braille version will ship by April.) We are asking each patron to return the order form within three weeks of receiving it. A copy of the contents of each format is attached for your information and to help you answer questions if any patrons call you about them.
Please do not take patron orders over the telephone or by e-mail. The CMLS contractor needs to receive directly the large-print or braille order form with the patron ID on it in order to put the request into the system. If patrons who have lost their order forms or return envelopes contact you, please notify Ingrid Davitt by e-mail at [log in to unmask] or phone (202) 707-0704. Ms. Davitt will need the name, address, and telephone number of the patron in order to replace the missing items.
Also, please do not use the attached informational copies to place orders for your library. Library order processes have been automated. Network libraries have the opportunity to update their first class (F/C) and bulk quantities and review their standing orders for regularly recurring publications on a quarterly basis. Refer to Network Bulletin 06-29 for details and instructions.
The CMLS codes for publications that patrons order will appear on your library’s CMLS records. After each publication is mailed, its code will be deleted from the records by the CMLS contractor. Transactions to add or delete these publications will appear on your library’s edit/update report.
