NLS requests that machine-lending agencies (MLAs) and network

libraries provide information about the number of machines repaired

during fiscal year 2012. Please complete the Book Machine Report in

the network library database by November 26, 2012.

The form is available from the Network Library Services website:                                      https://wwws.loc.gov/pics/. After logging on to the system, select                                                        the Network Database link. From this page select the Book Machine

Repairs link and ensure 2012 is selected as the year in the drop-down

menu. When reporting repair totals, please include those for C-ls,

DS1s, and DAls. The form is divided into three sections: C-1 Cassette

Book Machines, DA1 Digital Book Machines, and DS1 Digital Book

Libraries that are not involved with equipment repair should check the box at the top of each section to indicate no repair activities for this year, then click the Submit button at the bottom of the screen. MLAs also have access to the Repair screen for libraries in their service area and can submit repair information for them, if the libraries agree.
