SUBJECT   :    Revised Application for Free Library Service: Individuals

NLS has revised the application for library service for individuals. The notable changes are:

• Removal of cassette players, materials, and accessories
• Addition of a high volume player for readers with profound hearing loss
• Removal of requirements for receiving headphones
• Addition of instructions on how to find the local cooperating library where the application should be sent

NLS uses the application for individuals at numerous outreach programs. Network libraries that use the application may order revised copies through the WOW system. The order number is FR101. The online edition can be found on the NLS website at www.loc.gov/nls/pdf/application.pdf.
