Many thanks to all who replied on- and off-list.

There seems to be a slight preponderance of opinion to keep ‘Yoshida, KA” as the authorized access point (AAP).  As to what “KA” represents, this is unclear.  It seems fairly certain not to be the initials of the first and middle names, since Japanese names almost always consist of only one given name and a surname (family name).

One respondent, who claims only an “indirect” knowledge of Japanese, suggested that, since Japanese uses a syllabary, “KA” could stand for the first Japanese character of the name Keiko, the "A" being capitalized to indicate that both letters in English represent a single character in Japanese; he compared ToDa for Tokyo Daigaku rather than TD (granted that the capitalization is not the same).  This seems to be a plausible suggestion, although this usage would seem to be unique to Ms. Yoshida.

I have given cross references for both “Yoshida, K. A.” and “Yoshida, Keiko.”  “Yoshida, K. A.” may not be quite according to the rules, but it is clear from the citation that at least some people are unclear about how to interpret “KA,” and I suspect many others will also be.

     If KA is known to represent initials, then the correct form for the AAP would be: Yoshida, K. A.  This is defined in the LC-PCC PS for 1.7.1.  (Believe me; I’ve done this incorrectly at least once.)  See Access Points for Persons in Name Authority and Bibliographic Records; 1. Initials/letters.  If these are not initials, I suppose this rule does not apply.      I do not believe that a VAP from Yoshida, KA is consistent with the rule, but I could certainly be wrong about that.

I’d use the form that she using on the title page as the AAP (no space), unless there are other publications of her giving the name differently.

I seek the knowledge of the Collective Wisdom,

The current heading on NAR no2016036625 is Yoshida, KA, which I found a bit unusual, and there are no cross references.  She and her husband, David Mitchell, are the translators of Naoki Higashida’s The reason I jump.  In most of the occurrences of her name in the book, KA does seem to appear without spaces, although in About the translators, it sometimes seems as if there is a space, that is, as if she is using her initials without a space.

I found an article about her and her husband in The Telegraph, in which her first name is consistently given as Keiko.  The author of the article interviewed her husband at their home, so presumably this is first-hand knowledge.

So, my questions are, is KA some sort of shortening of Keiko?  Or should KA be taken as the initials of her first and middle names, and the heading be corrected to “Yoshida, K A”?  In any event, I think both forms, KA with and without spaces, should be access points, the only question is which should be the AAP?  And I also plan to add a cross reference for “Yoshida, Keiko.”
