I agree, NACO@LC wants the information to be clear but I have never seen an official template. Carl’s templates fit the bill. See the ones I use below, same

I have never seen strictly prescribed formats for reporting BFM, etc., to LC. I use something like this

to report BFM. (I find it easier to proofread the numbers in the message when I insert the dashes.)
When reporting duplicate authority records, I do it this way:
Hello NACO,
Please delete no2004-699992 ; it duplicates no2004-700003
Please delete no97-32009 ; it duplicates no93-10724.
I think the main thing is for the information to be very clear. If the folks at LC prefer something different, I haven’t ‘gotten the message’.
